It is a model upon which to base the design of an absentee application which an organization wishes to distribute within a particular county. If you are creating such a form for wide distribution within a single county, please note the segments where you will need to fill in specific data and, most importantly, the fact that your application must be 5-1/2" by 8" in size unless preapproved by the county elections official who will be processing it.
